Home Outreach Prevention & Engagement (H.O.P.E.)
H.O.P.E. is dedicated to inspiring hope, health, and well-being in an equitable way. We provide individualized care for people experiencing substance use, aiming to reduce overdose risks and support participants’ overall well-being through empowerment and comprehensive support.

Approach to Care
Our mission is to improve life outcomes for housed San Francisco residents who have experienced a non-fatal overdose. We work closely with the Street Overdose Response Team (SORT) and the Post-Overdose Engagement Team (POET) to help individuals live happier, healthier lives and reduce the risk of future overdoses.
Available Services
Participant Outreach
- Minimum of one phone call and two in-person attempts before removing a participant from the outreach list
- If direct contact is unsuccessful, a discreet note, voicemail, or text message will be left for the participant
Harm Reduction & Support Services
- Education on harm reduction practices
- Substance use support and treatment
- Linkage to counseling, psychiatric care, and primary care
Transportation Assistance
- Rideshare services (Uber, Lyft, etc.), including ADA-accessible vehicles, to facilitate attendance at appointments and access to community resources
- Note: HOPE staff do not transport participants in personal vehicles
Eligibility
To be eligible for the HOPE program, participants must:
- Have experienced a non-fatal overdose within the last six months
- Be living in permanent housing, a Single Room Occupancy (SRO) unit, or supportive housing
- Individuals experiencing homelessness will be referred to the POET program, which serves unhoused individuals who have recently experienced a non-fatal opioid overdose
How to Make a Referral
Referrals to the HOPE program primarily come from POET via the daily EMS/SORT log of overdose incidents in San Francisco County. Other referral sources include Emergency Department Substance Use Navigators, Patient Navigators, and staff from Permanent Supportive Housing.
