Project JUNO Community (JUstice-iNvolved Opioid use disorder treatment engagement in Community), is a 5-year SAMHSA grant funded patient navigator and contingency management program serving justice involved individuals started on buprenorphine (or newly methadone) while incarcerated in any California jail and assisting with initial linkage to either buprenorphine treatment at UCSF’s Office-based Buprenorphine Induction Clinic (OBIC) or methadone at San Francisco General Hospital’s Opiate Treatment Outpatient Program, or OTOP. Participants work with an assigned patient navigator for 6 months and will earn incentives for meeting with their navigator and working on substance use goals, such as taking their medication or reducing or stopping stimulant and/or opioid use.

However, our services are mobile, and we enjoy meeting patients in SF county jails, SF hospitals, and in the community. Intakes are Monday-Friday and typically occur at our office location or an arranged meeting place in the community.
Approach to Care
JUNO Community is committed to providing high quality, compassionate and evidence-based care to patients with OUD in San Francisco. Although we don’t prescribe medications directly, we work with are participants to overcome the multitude of barriers that prevent participants from continuing their medications for opioid use disorder post-release from jail. We continue to offer navigation services to our participants regardless of staying on buprenorphine or methadone assuring they have access to harm reduction resources to reduce the risk of overdose. We also offer rapid access to medication services through our partnerships with various clinics should a participant want to restart MOUD.
We also screen for the social determinants of health, like lack of transportation or insurance, and develop treatment plans that help address these challenges or barriers.
Available Services
- Patient navigation from jail to OBIC or OTOP, assistance with transferring clinics, residential treatment support, mental health and primary care (if needed).
- Contingency management for OUD and stimulant use disorder
- Motivational interviewing to help reduce problematic substance use
- Harm reduction and overdose prevention services and resources
- Transportation to and from appointments
Eligibility
- Must have a diagnosis of OUD
- Must be 18+ and staying in SF area (regardless of housing status)
- Must be *started* on Buprenorphine or Methadone while incarcerated in CA jails and will need to be verified
**We offer services in both English and Spanish and currently, our services are provided free of charge and no insurance billing occurs.
How to Make a Referral
We accept referrals from jail health services in our SF county jails, the OBIC & OTOP clinics, Addiction Care Team at SFGH, Pre-trial Diversion, and other community partnerships. Potential referrals can be made by calling Alexandra Haas at 628-206-3010 to check for program eligibility.
